Annual Health and Medical Forms  ( Link to BSA site for the latest form, will open in a new tab )

The Boy Scouts of America requires that Scouts and Adult Leaders have a current medical form on file and have annual medical checkups in order to participate in longer term camping activities.

Outings less than 72 hours require Parts A and B ( filled out by the parent or guardian ) that supplies essential medical details including health insurance information, contact, and any additional information that is required to be shared with the Adult leaders.  These forms travel with the troop on all outings.

Part C is required for all outings over 72 hours including summer camps at designated BSA facilities.  Camp Physicals need not be completed by the scout/leader’s regular doctor, as many urgent care centers ( Patient First, Righttime Medical ) and some local Health Specialty Stores ( CVS, Walgreens) provide summer camp physicals.
