Medical and Permission slips

Medical:

The Boy Scouts of America requires that Scouts and Adult Leaders have a current medical form on file and have annual medical checkups in order to participate in longer term camping activities. This form allows for first responders to treat scouts or adult leaders immediately if necessary.

Outings less than 72 hours require Parts A and B ( filled out by the parent or guardian ) that supplies essential medical details including health insurance information, contact, and any additional information that is required to be shared with the Adult leaders.  These forms travel with the troop on all outings.  

Part C is required for all outings over 72 hours including summer camps at designated BSA facilities.  Camp Physicals need not be completed by the scout/leader’s regular doctor, as many urgent care centers ( Patient First, Righttime Medical ) and some local Health Specialty Stores ( CVS, Walgreens) provide summer camp physicals.

Permission slips:

Permission slips are required for all scouting overnight activities. These must be signed by a parent or other guardian and they verify that they agree to let their child participate in the trip.
